1. Please fill in this form completely and click submit to send your reservation to Syosset Limo Service or Print this form, complete all information, and fax to SLS at (516) 364-3735. Keep a copy for your records.
2. Please RE-CONFIRM YOUR RIDE 24-48 HOURS IN ADVANCE, by calling SLS at (516) 364-9681, between the hours of 9:00 a.m. and 11:00 p.m. EST. SLS will indicate if a share ride has been arranged.
3. If you are delayed or cannot make your flight, please call SLS at (516) 364-9681 to let them know.
4. When you get to the New York airport please immediately call toll free 1 (800) 681-6668 (1 800 # only good in NYC area) for curbside pickup.
5. Note: In the event that your flight is delayed SLS will waive the share ride commitments. If you are aware of the delay, please notify SLS and they will try to coordinate another share ride. If SLS can not arrange a share ride for you, then they will have to charge you for a non-share ride.
